• Attach the third party request form and/or the OUSD Request for Income/Employment Verification Form
• Please note in the email, your dates of employment - This will help us facilitate the process

Income Verifications, Sick Leave Balance Transfer, and Retirement Forms

Income Verifications (for loans, mortgages, etc.), Sick Leave Balance Transfer and Retirement Forms (CALSTRS and CALPERS) should be directed to your Payroll technician.
