Meal Accommodation Requests
All sponsors of child nutrition programs are required to accommodate children with disabilities and children with special dietary needs whenever meals are served. OUSD Nutrition Services gladly supports these students.
To request an meal accommodation, download the form below, bring it to a physician to complete, then return it to your student's school. They will forward the form to OUSD's Health Services Department, who will then share it with Nutrition Services.

OUSD is a completely nut free district.
Our school meals do not include any tree nuts or peanuts, and all of our meals are prepared in nut free facilities in an effort to protect students with nut allergies.
