Meal Accommodation Requests

OUSD Nutrition Services is committed to accommodating students with medical disabilities, including food allergies, chronic diseases, and other medical conditions that affect eating. While we currently do not offer accommodations for dietary preferences, including religious restrictions or vegetarian/vegan diets, we strive to make our menu inclusive by offering plant-based options and ensuring that no pork is served.
New this school year (2025-26), we are proud to launch OUSD CARES (Customized Allergy Responsive Eating Solutions), which provides meals free of the top nine allergens to better support students with food allergies and other medical dietary restrictions.
To request a meal accommodation: download the form below, have a physician complete it, and return it to your child’s school administration staff and/or the school nurse. You may be contacted for follow-up as needed.
Please allow 2-3 weeks from the date of submission to the Nutrition Services dietitian for processing and coordination of OUSD CARES Meals.
Disclosure: OUSD CARES meals are carefully selected to be free from the top nine allergens and gluten. While we take great care to procure products that do not contain these allergens, manufacturer disclosures regarding shared equipment or facilities are voluntary under FDA guidelines. For added safety and to check for other allergens, we encourage families to review the ingredient and allergen information provided by the manufacturers. We are allergy aware. Our school meals do not contain peanuts or tree nuts as ingredients to support students with nut allergies.
Please note: while these allergens are not intentionally included in menu items, some products may be manufactured or processed in facilities that also process peanuts or tree nuts. Therefore, we cannot guarantee that meals are completely free from allergen cross-contact.
If your child has a severe food allergy, please notify your school and enroll in the OUSD CARES Program to request appropriate meal accommodations.
