Name and Gender Form
OUSD supports the rights of students to change their name and pronouns and have them consistently asserted at school (see administrative regulation 5145.3, page 3). Furthermore, changes can be made in Aeries with parent/guardian approval.
You can download the form here, print it out, sign and submit it to the Student Assignment Welcome Center. The Student (Assignment) Welcome Center is open to in-person visits by appointment. You can make an appointment online here or by calling, texting, or emailing us at 510-879-4600 and enroll@ousd.org.
Community Organizations
Oakland LGBTQ Community Center: Dedicated to enhancing and sustaining the well-being of lesbian, gay, bisexual, transgender, and queer (LGBTQ) individuals, our families and allies, by providing educational, social, and health related activities, programs and services.
Gender Spectrum: Gender Spectrum works to create gender sensitive and inclusive environments for all children and teens. Child, teen, and parenting support groups.
Trevor Project: National organization providing crisis intervention and suicide prevention services to lesbian, gay, bisexual, transgender, questioning/queer, intersex, and asexual (LGBTQIA) young people ages 13-24.
