Instructions for turning in Summer Program Documents
- Click on each of the files below and hit the "Download" button. Complete one of each document for each site where you will be running summer programming.
- Summer Program Planning Tool 2023 (if you would like a PDF version of this document click here) (Due March 17, 2023)
- Budget Template 2023 (to find the budget in an excel template that automatically calculates, download this document and save it to your computer to update) (Due March 17, 2023)
- Civic Center Permits / Facilitron (Due March 17, 2023)
- Nutrition Survey (6 hr Stand-Alone programs only. Not for A+B sites- principals will complete this survey.) If your agency plans to order snack from Nutrition services please complete this form and email to Donnie Barclift with nutrition services at donnie.barclift@ousd.org. (TBD)
- Summer Program Calendar (this is a sample document, feel free to use your own calendar template. (Due May 19, 2023)
- Summer Program Schedule (this is a sample document, please update with your hours and include your grade levels. (Due May 19, 2023)
- Summer Field Trip Schedule (Due May 19, 2023)
- Summer Directory 2023 (Due May 19, 2023)
- Summer Safety Plan (Due June 9, 2023)
- Summer Emergency Drill Report (Due June 9, 2023)
- For each agency level document listed below, complete one each for your entire agency and be sure to include your agency name in the file name.
- TB Clearance/Fingerprint Letter ( March 17, 2023)
- Statement of Qualifications (Due March 17, 2023)
- Certificate of Insurance (Due March 17, 2023) The Certificate Holder address is: Oakland Unified School District Attention: Risk Management, 1000 Broadway, Suite 440, Oakland, CA 94607 Please remember, on the Certificate of Insurance (ACORD 25 Form), the agency or organization must provide the following:
- Commercial General Liability Insurance & Worker's Compensation Liability Insurance
- OUSD must be listed as an additional insured either in the Description section of the form or on a separate Endorsement Page
- Register OR update your organization’s information in the Community Partner Process, our database of OUSD partners (Due March 17, 2023).
- Agency Signatory and Contact Survey ONLY ONE ENTRY PER AGENCY (Due March 17, 2023)
- Invoicing and Staff Qualifications Form (Due May 19, 2023)
- To submit documents, please upload them to your site-specific Google Folders, of which the link will be shared with you at a later date. To submit your Budget and Program Planning Tool documents, upload them to QUIP. You can access the QUIP website Here for additional details. When members of the Expanded Learning Team are reviewing your program documents we will use This Checklist. Please feel free to use this as an agency as you are preparing your documentation.
