Required Documents for Enrollment
The following completed forms and information are mandatory to enroll your child during your Registration Appointment:
- Birth Certification (Original) for all children counted in the family size
- Proof of Income for one month
- Proof of Residence for one month (e.g. utility bill or official government letter)
- Identification and Emergency Information- to be completed by parent/guardian
- Consent for Emergency Medical Treatment- to be completed by parent/guardian
- Student Disaster Preparedness- to be completed by parent/guardian
- Proof of Need for Each Adult in Household (e.g. Employment Verification, Proof of Seeking Employment or Seeking
- Housing, Proof of School/Training and/or Incapacitation Form)
- Pre-Admission Health History- to be completed by parent/guardian
- Health Examination Report- must be completed by physician
- Immunization Records- must be completed by physician
- TB Test Result for Child- must be completed by physician
- Asthma Action Plan- must be completed by physician (if applicable)
