Required Documents for Enrollment

The following completed forms and information are mandatory to enroll your child during your Registration Appointment:

  • Birth Certification (Original) for all children counted in the family size
  • Proof of Income for one month
  • Proof of Residence for one month (e.g. utility bill or official government letter)
  • Identification and Emergency Information- to be completed by parent/guardian
  • Consent for Emergency Medical Treatment- to be completed by parent/guardian
  • Student Disaster Preparedness- to be completed by parent/guardian
  • Proof of Need for Each Adult in Household (e.g. Employment Verification, Proof of Seeking Employment or Seeking
  • Housing, Proof of School/Training and/or Incapacitation Form)
  • Pre-Admission Health History- to be completed by parent/guardian
  • Health Examination Report- must be completed by physician
  • Immunization Records- must be completed by physician
  • TB Test Result for Child- must be completed by physician
  • Asthma Action Plan- must be completed by physician (if applicable)
