OUSD Open Enrollment is March 23rd - April 30th

What is Open Enrollment?

Open Enrollment is a one-month window (March 23 - April 30) each year during which employees can change their benefits coverage to be effective on July 1 (the start of the following fiscal year).

*Flexible Spending Accounts require re-enrollment! You won’t be automatically enrolled in any Flexible Spending Account (FSA) – you need to make an election to participate in an FSA each year. All other benefits elections do roll over from year to year.

Open Enrollment Benefits Options & Forms

How do I access forms?

  • Current retirees: To request forms, email benefits@ousd.org. Forms can be submitted via email or mail.
  • Current employees: Start and submit the appropriate forms in Frontline Central.  
    • Watch this 30-second Frontline Central Video, showing how to login and start a form, or
    • See the instructions below:  
      1. Log into Frontline Central.
        1. First, select 'My Forms' from the toolbar on the left.
        2. Then click the 'Forms I Can Start' tab.
        3. Finally, click 'Start this form' to the right of the form name (clicking on the form name will open a preview of the form).

Which forms do I complete?

Health Benefits

To change or enroll in Medical, Dental, or Vision benefits, complete this form:

  • Open Enrollment 2026: Benefits Election

If you elected Sutter Health, you must also complete:

  • Benefits: Sutter (Required if elected)

OR

If you elected Cash-in-Lieu, you must also complete:

  • Benefits: Cash-in-Lieu (Required if elected)

OUSD-Paid Life Insurance Beneficiaries 

To change your employer-paid VOYA Life Insurance beneficiaries, complete:

  • Life Insurance: Beneficiary (Required)

Flexible Spending Accounts (FSAs)

To enroll in pre-tax Flexible Spending Accounts, complete the appropriate form:

  • Open Enrollment 2026: FSA Election (Optional)

 

 

 

 

Supplemental Life Insurance

To enroll in VOYA Supplemental Life Insurance, complete:

  • Life Insurance: Supplemental Application (Optional)
  • Life Insurance: Evidence of Insurability
