    Field Trips

  • Use this page to find information about field trip policy and access scheduling forms.

    Lead agencies are required to complete the Schedule of Field Trips form twice during the year. One should be submitted on the first day of each semester, and you should submit one before summer programming- if applicable. Please see the attached schedule of field trips template and the field trip liability form to the right.

    The schedule should be submitted to your site's Field Trip deliverables folder.

    6. Field Trip Policy. FIELD TRIPS, OFF SITE EVENTS AND OFF SITE ACTIVITIES:

        • AGENCY shall provide each Site Administrator and the OUSD After School Programs Office with a schedule of all after school program field trips and/or off site events and/or off site activities by the first day of each semester, and a schedule of all summer field trips and/or off site events and activities by the first day of the summer program, if AGENCY is providing summer services (Exhibit D).
        • AGENCY hereby certifies that after school and any summer program staff and/or subcontractors will comply with the following procedures for all field trips, off site events and off site activities:

     

    6.1. Licenses Permission Slips/Acknowledgement. Field trip/excursion permission slip must be signed by parent(s)/guardian(s) of all student participants and an acknowledgement must be signed by all adult chaperones both of which shall include the following information:

     

    6.1.1. a full description of the trip and scheduled activities

     

    6.1.2. student/adult participant health information

     

    6.1.3. “Notice of Waiver of All Claims: Education Code § 35330 provides that all persons making a field trip or excursion shall be deemed to have waived all claims against any school district, charter school, or the State of California for injury, accident, illness or death occurring during or by reason of the field trip or excursion, regardless of who holds the claims. If the field trip or excursion to which this permission slip applies is out-of-state, I hereby knowingly waive all of my and my daughter’s/son’s/ward’s claims against any school district, charter school, and/or the State of California for injury, accident, illness or death occurring during or by reason of the out-of state field trip or excursion.”

     

    6.2. After school and summer program staff or subcontractors leading trip must have a written list of students attending trip.

     

    6.3. No student shall be prevented from making a trip due to lack of sufficient funds.

     

    6.4. After school and summer program staff or subcontractors leading trip shall have a sufficient first aid kit in his or her possession or immediately available. If the trip is conducted in areas known to be infested with poisonous snakes, this first aid kit shall contain medically accepted snakebite remedies.
