Feedback Form for Use with School Communities
School Improvement Grant Feedback Form - This form is a tool for parents and community members to organize opinions and feedback about the School Improvement Grant options. Forms should be collected by the facilitator at each site, and all feedback incorporated into the final report for submission to the Superintendent on April 14.
