Required Documentation

Please take some time to gather a folder with the following documentation for your child. The type of documents is going to vary depending on the child's previous enrollment status. See the requirements below.
 
A. New students:

A new student is a student that has never attended an OUSD school before. New students may be entering TK or K students, may also be students that are new to the area or students that are coming from a private or charter school, yet never attended OUSD.

If you are a new student you must bring the following documentation:

  1. Grades TK-1
    • Birth Certificate
    • 3 recent proofs of address* (see requirements below)
    • IEP if applicable
    • Parent/Guardian Valid photo ID
  2. Grades 2-12
    • Birth Certificate
    • 3 recent proofs of address*
    • IEP if applicable
    • Most recent Report Card/Promotion Certificate (grades 2-8) or HS Transcript (9-12)
    • Parent/Guardian Valid photo ID
B. Returning Students:
 
Returning students are those that have attended OUSD schools at any point between TK and 12th Grade. Students returning have a past record of attendance in our system. If your child is a returning student, please note these are the requirements:
 
  1.  Grades TK-5
    • 3 recent proofs of address* (see requirements below)
    • IEP if applicable
    • Parent/Guardian Valid photo ID
  2. Grades 2-12
    • 3 recent proofs of address*
    • IEP if applicable
    • Most recent Report Card (grades 2-8) or HS Transcript (9-12)
    • Parent/Guardian Valid photo ID
C. Current Students:
 Current students are those that are currently attending an OUSD school and wish to change schools before the beginning of the 2017-2018 school year. Current students must bring the following:
 
 Grades TK-1
  • ONLY IF MOVING RESIDENCE**: 3 recent proofs of address* 
  • Parent/Guardian Valid photo ID
Grades 2-12
  • ONLY IF MOVING RESIDENCE**: 3 recent proofs of address* 
  • Parent/Guardian Valid photo ID

*PROOF OF ADDRESS/RESIDENCE: 
 
You need to provide three current original documents that include the name and address of the parent/guardian. Any three of the following documents need to be dated within the last 45 days prior to enrollment:
 
  • Utility bills from agencies that require "physical"presence or hardware at address, such as PG&E, EBMUD, AT&T land line, garbage, cable, satellite TV. Bills "in care of" are not accepted.
  • Both automobile registration & auto insurance (count as one proof)
  • Homeowner's or renters insurance policy
  • Property tax statement, grant deed or title of property.
  • Lease agreement with the property owner's contact information
  • Official letter from a social service or government agency, such as IRS, EDD, Alameda County, etc.

BIRTH and CUSTODY VERIFICATION:

Birth certificates are the document we require for New students to prove age and in combination with the parent's photo ID to verify custody. In some cases we may ask for extra documentation or most recent court decisions. Some families may not have birth certificates, in the case of refugees/asylees provide the I-94. If you are currently in transition and are unable to retrieve the birth certificate you must contact us for further guidance.
Photo identification of the parent/guardian must be with a valid, official government issued ID card. We require Photo ID for parents of all students.
 
IMMUNIZATION REQUIREMENTS: 
 
New and returning students must provide immunization records to the school site once their assignment has been made. Information regarding immunization and health forms will be sent to the family's home along with the placement letter beginning in early March.
 
**DEFINITION OF RESIDENCY:
 
A minor's home residence is presumed to be the legal residence of the parent/guardian who have physical custody of the minor. To enroll in OUSD, said parent/guardian must reside within OUSD district boundaries or have an approved Inter-district transfer approval from their home district. A resident of OUSD is an individual who is a full time occupant of a dwelling located within OUSD boundaries.
